Welcome to Whole Heart Chiropractic

You made a great decision. This practice was built for people who have spent too long feeling dismissed, rushed, or invisible in healthcare. You will not feel that way here.

The intake process is designed to be simple, honest, and genuinely useful so that when you walk in, we can spend our time on what matters: you.

Right after you book

Your intake forms are on their way

Shortly after scheduling, you will receive both an email and a text to get started with your electronic paperwork. Depending on what we covered during your scheduling call, some forms may come right away and others will arrive closer to your visit. Either way, you will receive your health history and chief complaint survey 24 hours before your appointment so the information is as current as possible when you walk in. The forms live in ChiroUp, the secure patient portal we use for all records and communication. Most forms are pre-written - you just sign at the bottom. Only your intake forms (medical history, contact info) require filling out. 

Total time to complete 10-20 minutes

05Assignment of Benefits (if you are using insurance)
06Good Faith Estimate for Self-Pay Patients (if requested)
07Notice of Non-Covered Services (Medicare Patients Only)

Complete your intake forms at least 4 hours before your appointment. The health history questions are most accurate when you're not rushing. Giving yourself a little time means we start your very first visit with the clearest possible picture.

A quick note: There is no such thing as a "wrong" answer on your intake forms. The goal is simply to understand where you are right now, not to judge how you got here.

A first visit at Whole Heart is a conversation. We take the time to actually understand what's going on in your body before anything else happens. There is no rushing through to treatment.


Before you arrive

Complete your forms

Fill out your intake paperwork via our electronic health records at least 4 hours before your appointment. This helps us prepare specifically for you.


Day of your appointment

Pick the one thing that feels worst today

If you're dealing with chronic pain or discomfort in more than one area, we will ask you to pick the single spot that is bothering you most right now. This is not because your other concerns don't matter. It's because focusing on your most pressing issue first allows us to give you the most useful care on day one. Everything else gets its turn.


During your visit

Intake, exam, and your first adjustment

Dr. Jenny will review your history, perform an exam, and in most cases provide care on the same visit. You will leave with a plan that makes sense to you, explained in plain language.

Whole Heart Chiropractic is located inside Sage Studio Yoga and Wellness at 5133 S Campbell Ave, Suite 102, Springfield, MO 65810. 

If it is your first time visiting the building, look for Sage Studio on the lower level and come on in.

Check in at the kiosk

When you arrive, you'll check in using our self-service kiosk just inside the space. It's quick and easy. If you have any questions, someone will be close by to help.
